To apply to have your annual liquor licence fee reassessed.

Who can lodge this form?

You can lodge this form if you are:

• a licensee
• an approved manager
• a contact person
• an authorised legal representative.

What you need:

• Liquor licence details
• Applicant personal and contact details
• Applicant’s email address
• Information about why you want your fee reassessed including the details of the error in your assessment.
• Any supporting documentation such as a copy of your fee notice and documentation that shows evidence that your fee should be reassessed.
• A completed and attached authorisation consent letter if you are not listed as a licensee, approved manager or contact person on the liquor licence document.

• Make sure you take your time filling out this application correctly as you won’t be able to make changes once the application is submitted.
• If information is missing, processing this form will be delayed and the application may be refused.
• After submitting your application, you will be sent an email confirmation with a copy of your application. This will be sent to the email you provided on the form.

Please provide a clear explanation of the error in the amount you have been assessed to pay as indicated on your 2024 annual liquor licence fee notice.

Part 3 Declaration

• I declare the contents of this application, including attachments, are true, correct and complete and I have made all reasonable enquiries to obtain the information required to complete the application.
• I will immediately notify Liquor & Gaming NSW (L&GNSW) of any changes to the information in this application, if the information changes before the application is determined.
• I acknowledge that it is an offence to provide false, misleading or incomplete information in this application under section 36 of the Gaming and Liquor Administration Act 2007 and section 307A of the Crimes Act 1900.
• I acknowledge that failure to provide all required information may result in delay or refusal of this application.
• I understand that specific details I have supplied in this application may be 'personal information' under the Privacy and Personal Information Protection Act 1998. Personal information is any information or opinion that identifies an individual, or enables someone to identify an individual.
• I acknowledge that L&GNSW is collecting information to enable processing of the application. I also understand that L&GNSW will use the information for its intended purpose only, store the information securely, and allow the applicant or licensee to access and update the information. When processing this submission, L&GNSW may need to disclose information to other Government agencies.
• I understand that L&GNSW may use information provided in the application to inform further inquiries to ensure compliance with regulatory obligations.
